⭐ Squish, Scoop & Story & Recreational Center

I, [Parent / Guardian Name], am the parent or legal guardian of [Child Name(s)], and I wish to enroll my child(ren) in recreational programs offered by Squish, Scoop & Story & Recreational Center ("BARC").

Section 1 — Nature of Programs

I understand and acknowledge that all activities at BARC are recreational in nature. BARC is NOT a medical, therapeutic, clinical, or childcare provider and does not operate any before-school or after-school programs. Programs include, but are not limited to: dancing, painting, reading, music, yoga, arts & crafts, cooking, sensory play, and social skills activities. My child's participation is entirely voluntary and may be withdrawn at any time.

Section 2 — Health & Disclosure

I confirm that I have disclosed, or will promptly disclose, all relevant medical conditions, allergies, sensory sensitivities, behavioral considerations, and support needs to BARC staff. I confirm my child(ren) are medically fit to participate in recreational physical activities. I authorize BARC staff to seek emergency medical treatment for my child(ren) in the event I cannot be reached in a timely manner.

Section 3 — Assumption of Risk & Liability Release

I understand that recreational activities inherently carry some risk of physical injury. In consideration of my child(ren)'s participation in BARC programs, I, on behalf of myself, my child(ren), and my heirs, voluntarily assume all risks associated with participation. I hereby release and discharge Squish, Scoop & Story & Recreational Center, its owners, directors, staff, volunteers, and agents from any and all claims, demands, damages, and causes of action arising from or related to participation in BARC programs — except in cases of gross negligence or willful misconduct.

Section 4 — Photo & Media Release

I grant Squish, Scoop & Story & Recreational Center permission to photograph or record my child(ren) during activities for use in promotional materials, social media, newsletters, and program documentation. I understand I may opt out of this release at any time by notifying BARC staff in writing.

Section 5 — Attendance & Conduct

I agree to: (a) provide at least 24 hours' advance notice for cancellations or schedule changes; (b) ensure my child(ren) arrive on time for scheduled classes; (c) ensure my child(ren) follow BARC's behavioral guidelines, which prohibit aggressive, unsafe, or disruptive behavior. I understand that a parent or guardian must remain on-site during all BARC sessions and that BARC reserves the right to end a session early if safety or participation concerns arise.

Section 6 — Diaper Changing & Toileting Assistance

I understand and acknowledge that BARC staff may need to assist my child(ren) with diaper changes or toileting during program sessions. By signing this waiver, I authorize BARC staff to provide diaper changing and toileting assistance to my child(ren) as needed. I understand that:

(a) Diaper changes will be performed in a designated, clean area by trained staff following BARC's hygiene and safety protocols.

(b) I am responsible for providing all necessary supplies (diapers, wipes, change of clothes) for my child(ren). BARC is not obligated to supply these items, though emergency supplies may be available.

(c) Staff will follow standard sanitary practices including the use of gloves, proper hand washing, and sanitization of changing surfaces before and after each use.

(d) I will inform BARC staff of any skin sensitivities, allergies to diaper products, or special toileting instructions relevant to my child(ren).

(e) I release BARC from liability related to routine diaper changing and toileting assistance, including but not limited to diaper rash or skin irritation, except in cases of gross negligence or willful misconduct.

Section 7 — Indemnification & Hold Harmless

I agree to indemnify, defend, and hold harmless Squish, Scoop & Story & Recreational Center, its owners, officers, directors, staff, employees, volunteers, and agents from and against any and all claims, liabilities, damages, losses, costs, and expenses (including reasonable legal fees) arising out of or related to: (a) my child(ren)'s participation in any BARC program or activity; (b) any inaccurate, incomplete, or misleading information I have provided; (c) any injury or damage caused by my child(ren) to other participants, staff, or BARC property; or (d) any third-party claim related to my child(ren)'s presence at BARC. This indemnification obligation shall survive the termination or expiration of this waiver.

Section 8 — Duration & Revocation

This waiver remains in effect for one year from the date of signing or for the duration of my family's enrollment in BARC programs, whichever is shorter. This waiver may be revoked by providing written notice to BARC. BARC may require a renewed waiver if program policies change substantially.

Section 9 — Electronic Signature

By typing my full legal name below and submitting this form, I confirm that I have carefully read, fully understand, and voluntarily agree to all terms of this Participation Agreement & Liability Waiver. I acknowledge that my typed name constitutes my legal electronic signature. I understand that a signed PDF copy will be emailed to me for my records.
